Os representantes legais, ora subscritores deste formulário, autorizam expressamente o Contato Administrativo indicado acima a contratar e cancelar serviços de Cross Connection, em nome da Parte Contratante, com plena validade legal, os quais serão formalizados em formulário específico, conforme disposto na cláusula 4.3. do Contrato de Adesão ao Co-location, firmado entre A5X e a Parte Contratante.
The legal representatives, signatories of this form, expressly authorize the Administrative Contact indicated above to contract and cancel Cross Connection services on behalf of the Contracting Party, with full legal validity. Such services will be formalized through a specific form, in accordance with clause 4.3 of the Co-Location Adhesion Agreement executed between A5X and the Contracting Party.

Toda e qualquer contratação de serviço de Co-Location está condicionada à disponibilidade de espaço físico, energia e refrigeração no Data Center, bem como da formalização do respectivo contrato.
All Co-Location services are subject to availability of physical space, power, and cooling capacity in A5X’s Data Center, as well as the formalization of the respective contract.
Preenchimento / Completion
Este formulário, após devidamente preenchido e assinado por seu representante legal, passa a integrar o Contrato de Adesão ao Co-location firmado entre a A5X e a Parte Contratante, constituindo parte integrante e indissociável do referido Contrato, vinculando as Partes para todos os fins legais e contratuais.
This form, once duly completed and signed by its legal representative, becomes part of the Co-Location Adhesion Agreement entered into between A5X and the Contracting Party, constituting an integral and inseparable part of such Agreement, binding the Parties for all legal and contractual purposes.
A Contratante obriga-se a comunicar, por escrito, à A5X, no prazo máximo de cinco (5) dias, qualquer alteração dos dados cadastrais acima informados, sob pena de caracterização de infração ao Contrato de Adesão ao Co-location.
The Contracting Party undertakes to notify A5X in writing, within a maximum period of five (5) days, of any changes to the registration data provided above, under penalty of being considered a breach of the Co-Location Adhesion Agreement.
